Overview:Position: Lead RN Quality Performance Navigator
Hours/Shift: Full Time/Days
Minimum Requirements:

  • Education: Associate Degree in Nursing or graduate of a Nursing Diploma program
  • Licensure: Licensed registered as an RN in the State of Arizona
  • Certification: American Heart Association or American Red Cross Basic Life Support
  • Experience: Minimum of at least 1 year in a prior leadership role (manager, supervisor, lead, etc.) required.
  • Experience/Certifications: One year of practical experience with quality measures or Certified MACRA-MIPS Quality Professional; Advanced clinical assessment skills; Read and interpret medical records; Knowledge of HEDIS and CMS Star specifications
    • Preferences: BSN or MSN; Three yrs clinical or managed care experience; Previous experience educating patientsKey Responsibilities:Leads and supervise RN Quality Performance Navigators and coordinates population health wellness efforts. Performs Electronic Medical Record registry reviews in order to support all wellness and Quality Management (QM) activities:
      • Patient attribution and membership reconciliation
        • Examine membership profiles for accuracy
        • Verify third party payor and patient attribution integrity
        • Targeted workflow initiation
          • Proactively assign scheduling worklists to team members based upon key performance indicators and required metrics
          • Preorder approved protocol laboratories, request maintenance medications, and other population level targeted processes and procedures between and prior to office visits
          • Potential patient administrative correspondence as dictated by evolving workflows
            • Coordinate with the provider groups and clinical colleagues
            • Ensure required patient data is ordered, retrieved, and assigned appropriately for consumption by the primary provider at the time of service
            • Participate in the achievement of appropriate measured outcomes
              • STAR and HEDIS measures
              • Hierarchical Condition Category chronic problem documentation
              • Gaps in care and patient access
              • Payor Coordination
                • Consult with team on matters of third party clinical required metrics
                • Proactively review opportunities to improve patient outcomesKnowledge, Skills and Abilities: Excellent communication, organizational and interpersonal skills. Flexible and adaptable in a rapidly changing environment and attention to detail. Ability to interact and work effectively in a team setting.
